Primary Care Team
• Dr. Jill Caines, MD Dr. Caines attended Mount Saint Vincent University for her Bachelor of Arts Degree and Carleton University for her Masters Degree. She completed her Medical Degree at the Northern Ontario School of Medicine and Family Medicine Residency at Queen’s University. Dr. Caines resides in Lindsay with her husband, Mike, and her two young children. Dr. Caines works out of our Bobcaygeon office.
• Carrie Dymond, RN (EC) Carrie provides care to her patients in our Bobcaygeon office. Originally from the Burnt River area, she attended Fenelon Falls Secondary School and graduated from the Nursing program at Sir Sandford Fleming College and Trent University in 1999. Carrie continued her education at Humber College and Ryerson University, graduating as a Primary Health Care Nurse Practitioner from Ryerson in 2006.
• Dr. Alexandra Jacob, MD Dr. Jacob received her medical degree from the University of Sherbrooke, Quebec in 1989. After practicing for several years in Quebec and Ottawa, she is now working out of our Fenelon Falls office. Dr. Jacob is the mother of three sons and is excited to be living in the City of Kawartha Lakes. “I am looking forward to this next phase of my life and hope to contribute to serving the medical needs of the community. My family is looking forward to visiting and enjoying the beautiful lakes and rivers of the City” .
• Dr. Luke Johnson, MD. Born in Canada, Dr. Johnson received his medical training in Australia. Since 2016, he has worked as a general practitioner on the Coffs Coast in New South Wales. He received his Bachelor of Science (Honours) from Carleton University in 2009, his Bachelor of Medicine Bachelor of Surgery (MBBS) from the University of Wollongong in New South Wales in 2013, and his Fellowship Royal Australian College of General Practitioner’s (FRACGP) in 2018
• Agnieszka (Agnes) Okoniewska, RN (EC) Agnes provides care in our Minden Urgent Care Clinic
• Amy Thompson, RN (EC) Amy has been providing nurse practitioner services from our Fenelon Falls office since 2007. She completed her nursing training in North Bay at Canadore College in 1997 and completed her Primary Care Nurse Practitioner certificate in 2006 at Ryerson University. She added a certificate in Gerontology from St. Xavier University in 2012 and her Masters in Nursing with a focus on teaching in 2017.
• Dr. Hor-Ven Zung, MD Dr. Zung received her medical degree from the University of Alberta in 1989 and completed her Family Medicine Residency at the University of Saskatchewan. She practiced family medicine in Alberta before moving to the City of Kawartha Lakes to start “Living the Dream” on Sturgeon Lake, she practices out of our Fenelon Falls office.
